This telemedicine project leverages innovative smart technology to improve cardiovascular health management in primary care. Cardiovascular diseases (CVDs) pose a significant burden on the healthcare system, and proactive, personalized care is essential to reduce this impact. This telemedicine project explores whether a home monitoring system equipped with tools like a blood pressure monitor, weighing scale, and activity tracker, can enhance self-management and health outcomes for patients at high risk of CVD.

The study is designed as a non-inferiority trial, comparing standard care with the use of this telemedicine innovation. Participating patients use the technology for health monitoring and share data with their general practitioner through a secure app. This proactive approach aims to improve health awareness and encourage lifestyle changes to mitigate cardiovascular risks.
