Empowering healthcare providers to refer patients with chronic illnesses to tailored support networks can improve digital inclusion.

This project addresses the growing need for equitable access to eHealth, focusing on patients with chronic illnesses such as cardiovascular diseases, asthma, COPD, and diabetes mellitus. Despite the potential of digital healthcare technologies to improve outcomes, many patients face barriers due to low eHealth literacy and inadequate support networks. By empowering healthcare providers and societal organizations, this initiative aims to bridge the gap in digital inclusion, ensuring that all patients can benefit from eHealth advancements.

Through a patient-centered and collaborative approach, the project investigates how social and medical networks can support patients in adopting and using eHealth. This includes identifying barriers and opportunities, integrating insights from patients' social networks, and co-creating practical solutions with healthcare providers, patients, and support organizations. Using the Double Diamond Model for design thinking, the project moves from understanding needs to implementing sustainable support systems.

The goal is to create a clear tested roadmap for how eHealth support can be integrated in general practice. This roadmap will help healthcare professionals connect patients with tailored support networks, making it easier for everyone to access and use digital healthcare. It creates a scalable and inclusive framework for digital healthcare adoption. By focusing on collaboration and innovation, the aim is to make digital healthcare accessible to all, improving health and reducing inequalities.
