The HAMLETT Study

People who experience a first psychotic episode are usually prescribed antipsychotic medication. We know that these medications are effective in treating psychotic symptoms and preventing a subsequent episode.

Current guidelines therefore recommend continuing these medications for at least one year after psychotic symptoms have disappeared. However, there is recent evidence suggesting that reducing antipsychotic medication earlier may have beneficial long-term effects on functioning.

The HAMLETT study investigates whether the current guidelines provide the best outcome, or whether it is better to start tapering antipsychotic medication earlier. We have also developed a tapering guide to help participants safely reduce antipsychotic medication.

The OPHELIA Study

The 10 years following a first psychotic episode are crucial for the development of a person’s mental health. Participants from the HAMLETT study are followed during this critical period in the subsequent OPHELIA study. The study focuses on three important aspects:

  1. Existential and personal values of the individual
  2. Natural variation in treatment
  3. Genetic predisposition and the interaction between genes and environment 

The OPHELIA study aims to understand which factors positively or negatively influence the course of a person’s mental health. With this information, tools can be developed to better tailor treatment for psychotic disorders to individual needs.

Who is participating

Since 2023, the inclusion phase of HAMLETT has been closed. Over the coming years, we will continue to see our participants regularly for study visits. Participants in the study meet the following criteria:

  • Men and women aged 16–60 years
  • Individuals who have shown improvement with antipsychotic medication 3 to 6 months after a first psychotic episode
  • Individuals who are currently still using antipsychotic medication
  • Able to understand the Dutch language
  • Capable of making an informed decision about whether or not to participate
  • Simultaneous participation in another medical-scientific study on the effects of medication is not possible
