Shared care and cardiovascular risk management for testicular cancer survivors

 Combining clinical practice with research, I work in the outpatient clinic and contribute to the T-CARE study. This implementation research focuses on shared care between hospital specialists and general practitioners to optimize cardiovascular risk management for testicular cancer survivors. My aim is to support closer collaboration between hospital and primary care to improve long-term survivorship care.
