Association of diuretic use with increased risk for posttransplantation diabetes

Post-transplantation diabetes mellitus (PTDM) is a major clinical problem in kidney transplant recipients (KTRs). Use of anti-hypertensive drugs such as diuretics has been known to be associated with increased risk of new onset of diabetes in the general population.

Diuretics are frequently prescribed in KTRs as the initial choice of antihypertensive medications and volume optimisation.

We explored the associations between diuretic use with incident PTDM in KTRs. We found that use of diuretics is a strong and independent risk factor for incident PTDM. Our finding highlight the association of thiazide diuretics and particularly loop diuretics with increased risk for PTDM.

We identified diuretic use as a potentially modifiable risk factor for PTDM development. These results call for careful evaluation of the necessity of diuretics use in KTRs.
