Seven out of 10 hospital beds across Europe are occupied by people with chronic long term conditions. Outcomes in these patients will improve by integrating our health and social welfare systems to provide long term care. That’s where CONNECARE comes in.

Connecare is a large EU consortium that has demonstrated the benefits of an IT management system to support integrated care delivery in chronically ill patients. This system has resulted in significant improvement in patient health outcomes. Compared to care-as-usual, it reduces healthcare costs by preventing unnecessary hospitalisations, thereby creating value-for-money.
